Application Process

Who is eligible? Children (up to age 18) that were involved in lawn mower or farm machinery accidents.

What will we approve? Medical expenses, prosthetics, and safety material.

Please complete the online application and submit paper documents within 7 days of application. Mail all documents to Tate’s Army Foundation at:

Tate’s Army Foundation Board of Director’s will review and approve all applications. We reserve the right to ask for additional information, including detailed insurance information, tax return information or follow-up conversation on the application.
